Clinical Pharmacist

Valentine Health Partnership is a large state-of-the-art GP training practice located in the diverse and multicultural borough of Greenwich. Serving approximately 33,000 patients across its two sites in Woolwich and Shooters Hill, the practice is committed to health promotion and disease prevention. This is an exciting opportunity for an enthusiastic, highly motivated, and experienced Clinical Pharmacist to join a dynamic and forward-thinking team. The successful candidate will work alongside pharmacists and technician colleagues within a larger multi-disciplinary team, providing clinical pharmacy support to the practice and addressing prescription and medication queries. By fostering integration between community and hospital pharmacy services, the Clinical Pharmacist will play a crucial role in improving patient outcomes, optimising medicines use, and managing patients with long term conditions.

The post holder will provide patient-facing services within the practice, handling both simple and complex inquiries from patients, carers, and third parties regarding medications and long term conditions. They will conduct medication reviews, including annual assessments, and monitor patients with complex polypharmacy, ensuring that systems are in place to recall patients as needed and that risks associated with ongoing drug treatments are managed effectively. As a key member of the team, they will support GPs and practice nurses with long term condition management, prescribing, and medication optimisation, while also managing their own caseload of complex patients.
