Primary Care Network Paramedic
Riverview Primary Care Network

The post holder will be employed by Greenwich Health, the GP Federation in Greenwich. Greenwich has 7 primary care networks (PCNS) with list sizes of between 30-80,000 patients. The Post holder will work for Riverview Primary Care Network, a grouping of 5 practices.

 

Paramedic Practitioners (PPs) - are paramedics who have undergone additional education and training to equip them with greater patient assessment and management skills. They are able to diagnose a wide range of conditions and are skilled to treat many minor injuries and illnesses .PPs are also able to signpost care referring patients to specialists in the community such as GPs, community nurses or social care professionals. The PP will demonstrate critical thinking and is a caring, compassionate and committed, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Riverview Primary Care Network

The post holder will be employed by Greenwich Health the GP Federation in Greenwich. Greenwich has 7 primary care networks (PCNS) with list sizes of between 30-80,000 patients. The Post holder will work for Riverview Health PCN, a grouping of 5 practices. 

A GPA will support the GP(s) by support clinicians with a variety of admin tasks reducing the clinicians administrative burden.

Your normal hours of work will be between 8.00 a.m. to 8.00 p.m. Monday to Saturday.

Primary Care Network Senior Clinical Pharmacist
Riverview Primary Care Network

The post holder will be employed by Greenwich Health the GP Federation. The Post holder will work with Riverview Health PCN, a grouping of 5 practices. Riverview has a team of clinical pharmacists working to deliver practice support but also PCN level work which means the individual pharmacists are part of a wider team.

The post holder will work as part of a multi-disciplinary team in a patient-facing role and  proactively manage patients with co-morbidities.

Provide support to general practice staff with regards to prescription and medication queries. Support the repeat prescriptions system, deal with acute prescription requests, and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practices.

Provide leadership on medicines optimisation and quality improvement and manage some aspects of the quality and outcomes framework and enhanced services.

Primary Care Network Physicians Associate
Riverview Primary Care Network

The post holder will be employed by Greenwich Health the GP Federation in Greenwich. Greenwich has 7 primary care networks (PCNs) with list sizes of between 30-80,000 patients. The Post holder will work for Riverview Primary Care Network, a grouping of 5 practices.

The post holder will work as part of a multi-disciplinary team in a patient-facing role.

The post-holder will undertake clinical sessions within the Primary Care Network, dependent on patient demand and the needs of a practice. The post holder will be aligned to a maximum of three practices to enable sufficient relationships to be built with their clinical supervisor.

Primary Care Network General Practice Assistant
Valentine Primary Care Network

The post holder will be employed by Greenwich Health the GP Federation in Greenwich. Greenwich has 7 primary care networks (PCNS) with list sizes of between 30-80,000 patients. The Post holder will work for Valentine Health Partnership, part of Valentine Primary Care Network.

 

A GPA will support the GP(s) by support clinicians with a variety of admin tasks reducing the clinicians administrative burden.

Your normal hours of work will be between 8.00 a.m. to 8.00 p.m. Monday to Saturday.

Valentine Primary Care Network

The post holder will be employed by Greenwich Health, the GP Federation in Greenwich. Greenwich has 7 primary care networks (PCNS) with list sizes of between 30-80,000 patients.The Post holder will work for Valentine Primary Care Network.

An ACP will demonstrate critical thinking and is a caring, compassionate and committed experienced nurse who, acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

 

They will demonstrate safe, competent clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice. They will communicate and work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
